Initial Comments:
Based on the findings of an offsite followup survey completed on August 2, 2021, Vna of Northwest Pa, Llc, was found to have corrected the deficiencies cited under 42CRF, Part 418, Subparts A, C and D: Conditions of Participation: Hospice Care. The deficiencies were cited as a result of a Medicare recertification and state relicensure survey completed June 10, 2021.
